Step 3: Getting Started
Based on your answers, we may be able to help you regarding your complaint. Have you already talked to the public body? If possible, we recommend trying resolve your complaint directly with the organization before contacting us.
You’ll need to tell us:
- Who your complaint is about
- What your complaint is about
- Basic personal information
There are three steps to complete the form, and it takes about eight minutes to fill in. You can attach documents as evidence to support your complaint or they can be provided at another time.
The form will only be saved once you select, Submit Form.
Once you submit the form, our intake team will review your information and contact you to discuss next steps. If you prefer to call or send a letter, visit our ’How to Complain’ page for more ways to complain.
