Investigation Report: PHIA Collection – Health-care Facility

Summary

An individual attended a follow-up appointment at a hospital’s day surgery department. As part of the check-in process, she was asked to provide her religion for the admission record. The individual felt that the collection of this information was unnecessary and objected. We found that collecting information about the complainant’s religion was not necessary for health care and upheld the complaint. The hospital agreed to adopt procedures to limit the collection of this type of information to circumstances that may be related to the provision of spiritual care to patients.
