Ali Al Taki fell and struck his head after being pushed by another patient at the Selkirk
Mental Health Centre. While receiving treatment for this injury at Health Sciences Centre,
he contracted pneumonia due to his immobility and died on June 24, 2016. Status of review –
completed November 5, 2018. As there were no recommendations for the ombudsman to monitor,
we did not issue a final report to the provincial chief judge. We notified Manitoba Health
Seniors and Active Living of our receipt of the inquest report and closed our file.
