How to Complain
What information to provide in your complaint:
- The name of the public body (organization, program, department, agency, municipality or health-care organization) you are complaining about
- A summary of your complaint with enough detail to explain the problem, including actions you have already taken
- Important dates and timelines about your complaint or disclosure
- Names and phone numbers of public body employees you have been in contact with about your complaint (e.g. manager, supervisor, designated officer, or other)
- Information about any appeal hearing that may have been held and the outcome (if relevant)
- Copies of any related documentation
- Your full name, address and phone number where you can be contacted
